Diabetes Personal Action Towards Health (PATH)
Diabetes PATH is a six-week workshop that introduces self-management tools to manage diabetes and prevent complications. Topics covered include healthy eating, physical activity, monitoring blood glucose, and communication with healthcare providers. Participants will receive a free diabetes self-management book and gift upon completion of the program.
